Over the past quarter, the HealthPathways team has continued to support clinicians across the Murray PHN region through ongoing pathway development, review and refinement. This edition highlights key updates and opportunities to be involved.

Pathway updates

The following new pathways have been developed over the past quarter to support clinicians with up-to-date locally relevant guidance:

Over the past six months we have reviewed several key pathway (suites) including:

  • Refugee Health Care
  • Refugee Health Assessment
  • Thromboprophylaxis in Pregnancy
  • Osteoarthritis (OA)
  • Weight Management Specialist Referral
  • Joint Hypermobility
  • Postural Orthostatic Tachycardia Syndrome (POTS)
  • Bone and Joint Infections in Children
  • Genetic and Genomic Testing in General Practice
  • Pigeon Toes (Intoeing) in Children
  • Popliteal (Baker's) Cyst in Children
  • Curly Toes in Children
  • Metatarsus Adductus in Children
  • Flat Feet (Pes Planus) in Children

Since the introduction of individualised accounts in April 2025, 1,703 primary care professionals have registered for access to Murray HealthPathways.

Priority areas

Participate in Clinical Working Groups on older persons

Murray PHN invites local GPs and nurses to join online Clinical Working Groups (CWGs) focused on improving pathways for older persons.

Topics:

'Medication Management & Polypharmacy in Older Adults' - Thursday 18 June

'Driver Assessment of Older Persons' - Thursday 25 June.

Format: Microsoft Teams, 6.:30–7.:30 pm.

Facilitated by the Project Lead HealthPathways Dementia and Aged Care the sessions will improve referral pathways and capture local insights. Eligible self employed or private health professionals are remunerated and places are limited.

Dementia Survey for GPs

The HealthPathways team are conducting a GP survey to understand the barriers and enablers of optimal care for patients with mild cognitive impairment or dementia.

Supporting people living with dementia in our region

Murray PHN has developed a range of dementia resources and a local service directory to help people living with dementia, their families and carers.

The online directory brings together key services such as Cognitive, Dementia and Memory Services (CDAMS), carer support groups and respite options, making it easier to find help close to home.

A suite of consumer resources is also available, including information sheets and booklets in multiple languages.

Service updates

Are you receiving referrals from GPs and other health professionals? Is your service information up-to-date in HealthPathways?

HealthPathways ensures:

• Better quality referrals into your programs – ensure GPs and other health professionals know what your service offers, who is eligible, information needed etc.

• Increased visibility across the region – chance to promote your services

• A single trusted place to keep service information accurate

• Reduced administrative burden – minimise incomplete and inappropriate referrals, and back and forth correspondence to obtain required information

• Control how your services are understood, especially lesser known or new programs and clarify misconceptions.

Automated emails have also been introduced for service updates. Relevant regional services will receive a link to where current service information is listed and will be asked to review and reply within two weeks.

Refugee Week 2026-New Refugee HealthPathways

During Refugee Week (14–20 June), we recognise the strength and resilience of people from refugee backgrounds in our communities.

To support clinicians in delivering safe, culturally responsive care, Murray HealthPathways has introduced two new pathways:

  • Refugee Health Care
  • Refugee Health Assessment

These pathways bring together practical guidance on initial health assessments and screening considerations, helping clinicians take a structured and informed approach to care.

By making this information easily accessible at the point of care, HealthPathways supports timely, coordinated, and appropriate care for people from refugee backgrounds.
